| PRESCRIPTIONS REFILL AND UPDATE OF MEDICATION LIST AT THE TIME OF VISIT | |
| | | |  | | If you need a prescription medication refilled, even if it is written “No refill” on your bottle, please ask your pharmacist to fax us a request and allow 24 hours to complete the process. Avoid waiting until you are at your last pill to call your pharmacist. Remember that controlled substances (Some pain medication) cannot be refilled by phone and that you will need an office visit. Many insurance plans have special rates and requirements usch as mail orders, 90 days prescriptions and patient assistance programs. It is to your advantage to read about the services offered by your insurance provider. You must bring all your medication and non prescriptions (over the counter and supplements) at the time of each visit. These are needed for review of the refills and also to make sure your medical chart is up to date with any additional medications recommended by another Physician.

| REFERRALS & AUTHORIZATIONS | |

| You are responsible for making sure that you have an authorization to see any specialist or to receive ancillary services. Please call us as soon as you have an appointment, and allow us at least 72 hours to obtain the authorization from your insurance company.
In general, the appointment must be made first with the specialist and we also need the date of the visit.

| END OF LIFE ISSUES | | Dr Laramee has obtained her Board Certification in Hospice and palliative Care Medicine in February 2007.

| | | Dr Laramee has been involved with The Hospice of the Florida Suncoast since 2003. She is now the team physician for one of the Hospice teams and a volunteer of the pet therapy program with her Leonberger, Daisy Bear, who has been certified to be an Animal-Assisted Activity Pet Partner by the prestigious Delta Society .
She is a firm believer that end-of-life issues should be addresses early in life, starting with advanced directives. Please feel free to discuss your questions and concerns with Dr. Larmee.

| FEES & PAYMENTS
| | Payments for services is due at the time of your office visit, including co-payments and fees, deductibles, non-covered services and balance due. Cash, Checks and major credit cards are accepted. You are responsible for any balance not covered by your plan.

 To avoid unexpected charges, please review your insurance plan to understand the services that are covered before your visit. Medicare and many insurance plans do not cover preventive medicine tests and services for which you may be billed.
Medicare has a list of covered services and we encourage you to refer to the website Medicare.gov for more information. Several third party payers are modeling their service on the Medicare benefit program.
Make sure that you notify us regarding any changes in your insurance coverage Feel free to call us with any billing questions.
